Parent Inventory
Everyone · Everywhere · Experiencing Jesus
As parents, you have the awesome responsibility of being the primary disciple-maker in your child's life. This inventory is a guide — not a grade. It helps you see where your child is on their journey toward Jesus so you can love and guide them more wisely. The goal is not simply salvation. It is a full follower of Jesus — someone for whom faith is not a label or a childhood memory, but a living daily reality.
"Train up a child in the way he should go; even when he is old he will not depart from it." — Proverbs 22:6
A note for every family: This tool assumes a child can express their faith and questions in fairly typical ways. If your child communicates or experiences the world differently — because of a disability, a diagnosis, or simply how God made them — this inventory may not fit well, and that is not a reflection on your child or your parenting. We would rather talk with you directly than ask you to force answers into a format that was not built with your family in mind. Please reach out to your children's or student ministry leader — we are working on a fuller way to think and talk about this together.

Before You Begin
A few questions to help personalize your results.
A word before you answer: Try to answer based on what is consistently true of your child — not their best moments, and not what you hope is true. A quiet child with genuine faith may score differently than an expressive child with performed faith. Both patterns are worth seeing clearly. And a word on the youngest ages in particular: this inventory describes formation indicators we can observe. It is not making a claim about a fixed "age of accountability" or when a child becomes capable of salvation — that timeline is God's to write, not ours to calculate.
Please fill in your name, your email, your child's name, and their age range before continuing.

Questions Your Child Is Asking
The Wondering Inventory
Check any questions your child has asked — in any form, in any setting — in recent months. These are among the most honest formation indicators available. Children ask what they genuinely wonder. You do not need to have answered these perfectly. Simply check whether your child has asked them.
Section One
Doing
The visible practices and behaviors of faith — what your child does, and whether it seems to flow from their own initiative rather than only compliance.
Section Two
Being
The interior formation beneath the behavior, across five domains — Spiritual, Relational, Emotional, Intellectual, and Physical. Answer from what you genuinely know, not just what you observe on Sundays.
Section Three
Fruit & Expression
How your child's formation — or the early signs of it — shows up in how they treat people, handle difficulty, and move through the world. The outward evidence of interior reality.
Seasonal Overlay · The Wall (Stage 5)
My child is currently in a season marked by significant difficulty, loss, trauma, or grief — such as the death of someone close, serious illness, family crisis, or an experience of injustice — that has noticeably affected their faith or relationship with God.
Heritage Observation · Faith from the Beginning
My child has been immersed in a faith community and exposed to the gospel from a very young age — they have never known life outside of a Christian home and church environment.

This inventory is an observation tool — a starting place for conversation, not a final verdict. Formation is God's work; your role as a parent is to cooperate with what he is already doing.

Bring these results into a conversation with your pastor, a trusted mentor, or your child's ministry leader at Fellowship. You were not designed to do this alone.
